When you're old enough and want kids of your own you take a parenting class and pass with perfect scores and have to pass a sanity test. If you pass then you get a shot that makes you fertile again and you can have kids. Those who fail can't breed.
Once you have kids, social services will visit you randomly for 3 years to make sure the kids are safe and getting the care they need. If you're caught abusing them, you become sterile again and the government will never allow you to have kids again
