If you actually pay attention during the movie after he finishes talking to harvey dent the joker takes both hands off the gun meaning harvey could shoot him. That's when dent flips the coin.

ok so i have limited texts and i texted my friend and she texted me, "oh, using up those texts, are you now," so i quoted the joker and said, “It's not about the money, it's about sending a message."

I am not 100% sure I understand the exact mechanics of the hammer on a revolver, but I am pretty sure it'd still shoot. The action of pulling the trigger normally makes the hammer fall at a high speed, so I think that holding it back would just tear the Joker's finger off, while putting a bullet through his brain.
Though like I said, I am not exactly sure how the trigger-hammer system works on a revolver.
