I cant tell if its 2D painted to look like 3D or if it's actually a 3D painting

My personal guess is that it's 3D and acrylic, but I'm no expert.

It’s either oil or acrylic. Oils are easier to do in this style but take much longer to dry. And yes, it actually stands up off the canvas.
