I had a super bad cough once and I had some strong cough syrup I thought the prescription said 15ml but it said 5ml. I was so dizzy and felt like my eyes were zooming in and out. That was a horrible day at work.
Similar, but reverse once happened to me.
I was prescribed liquid Tylenol 3 after throat surgery and in my post surgery and anesthesia addled state I misread the dosing and took two teaspoons. I was miserable to say the least.

After two days, I reread the dosing and realized I was supposed to be taking two tablespoons... things were a lot better once I was dosing myself properly.
