In my experience it's to do with the shape of your ear. There's probably a little flap that most people have but you don't, and it makes all the difference.

My ear canals are scarred so I cannot get these in my ears without physical pain.

Ok so there are different sizes of earphones. You need the smaller size. If you want them in deeper twist it a bit
