Most state schools have uniforms too, although there is no legislation governing school uniform. Sorry for the redundancy in the earlier GCSEs explanation, same over informed guest here.

Nope. Formal wear isn't at all necessary. For example if you are registered for the exams via your school then you must be dressed in your school uniform and nothing else is acceptable. But for example if you're a private candidate say for example someone who decides to take an exam again to improve their grade after having graduated from school then you can dress in whatever clothes you would want to wear. Most private schools have a uniform and thus you wear one when taking the exam because technically speaking you're taking the exam to graduate said school.
