The kittens were in a burning building, they weren't the ones actually burning. They were probably in a corner somewhere meowing their little heads off.
Fire injuries take an incredibly long time to heal so there's no way the picture was taken right after the fact, the mama cat was probably at death's door then. Any side effects the kittens suffered from the fire would have healed by the time the mama was allowed rejoin her babies. I have seen fire damaged animals that kept going back in the fire for whatever reason (I volunteer at very overcrowded rescue shelter) and they don't always have a happy ending, but endings like this do exist.
