I did this once in Disney. I was nine. My family left me standing at the door and continued walking.

After a couple of minutes make eye contact with someone going through the door. Smile and glance at the door, taking an extra moment to make sure they have contact with it. If needed, you can say "got it?". Works for me every time. Avoids lots of awkwardness.
A couple of minutes? That's a long time to hold open a door. I always only hold it open for the first person unless they're old or a kid and then pass it off.

A couple of minutes is my comfort zone when I'm not in a rush. Adjust the time frame to fit your own needs and comfort.
