Although, the child is fairly ambiguously gendered. Plus, that heart is a bit too big. I think the mother should get that checked out.

There's probably a price tag on the heart to. it represents how much they give to the child even though you can't put a price on a brain or heart

It might be an identification tag, like the ones that coroners sometimes probably use.

When I saw the original image, I interpreted this differently. Sometimes parents want you to think and feel exactly how they feel and want you to be a copy of them. And when you try explaining to them that you are different, they just don't seem to understand it.
