Well, when you consider how many doctors take care of patients each year, and how many patients that are cared for that die each year, it's not that unlikely that 7000 are dead because of misread instructions, prescriptions, etc.
It's the same as a lot of people die because of chocolates.
Their precondition play MUCH GREATER part in death. Chocolate was there to trigger it.

Even so, at least some people die from overdosing by doctors orders when they would otherwise be ok.
I'm not saying don't go to the doctor, doctors in the whole do a lot more good then harm, however especially from a doctor I think there is a much needed additute change here.
What I am saying is that doctors should be mindful of the fact that mistakes could cost lives, and act accordingly. And hospitals should do their best to make policies that minimise risks, such as typed persriptions to avoid misread handwriting.
There will still be accidents of course, but the most we can do to improve medicine and minimize those accidents, the better.
