you end up with this big empty world and the occasional side mission

I just deleted all 14 days, 17 hours, and 32 minutes of saved data. Now I'm going to beat the game without any of the factions turning on each other. Institute was decent

Yep, its actually scripted in the game, but you have to be careful not to complete specific quests.
