Really I think both situations are none of your business to bring up to someone unless you're close friends or immediate family that truly cares about their well being. I think especially with something like fat shaming, people use the excuse "well its unhealthy" but really don't even care about their health or well being. They're just looking for a reason to put someone down.
Both smoking and being obese are lifestyle choices that are unhealthy. Both types of people likely know it's bad for them, they don't need you pointing it out when you don't actually care about their well being. There are multiple reasons for being obese and smoking is an addiction and like any addiction, it's really hard to quit.
I don't smoke at all. I never want to. Maybe you stop jumping to conclusions about me. As someone who is overweight and has struggled with a food addiction my entire life, I'm trying to change that and it's one of the hardest things I've ever done. It makes me mad when people are like "well just stop eating so much". It's not as simple as that and I know smoking is the same way because I have multiple family members that struggle with it. While it's one thing for loved ones to encourage a healthier lifestyle it's not okay for you just go up to anyone and tell them they need to fix their problems. It's an addiction and it's hard to overcome. I'm just saying it's nobody else's business and quit trying to tell people "oh you need to do this because what you're doing now is unhealthy" when you're someone that doesn't care or hardly knows the person.

People DO care about other's well being. I for one hate when either situation shows signs of hurting someone in a physical or mental way ie addictions or massive weight gains and I WILL say something to help you get over it whether you want my help or not. Other people are like this too, not everyone hates everyone as much as you say they do.
(That came off super aggressive; TLDR:) As a medical student I do care about people and their business with their health, I'm not all in their faces yelling EAT RIGHT or STOP SMOKING but I do have conversations about these issues with those that have it almost everyday.
Ok well it's one thing if you're working in a medical office with a doctor or working as a doctor. I'm talking about people like Nicole Arbour. I don't know if you've seen her video called Dear Fat People, but she makes the same argument about that it's unhealthy but I'm not convinced she legit cares about people's health. It's people that think it's okay to just butt into everyone's business about their life choices regardless of their relationship with that person. Do you see what I mean now?
And sometimes it gets to the point that person well knows what they're doing is bad for them, but they have to find the willpower to change it their own. I had people telling me for years about my weight being an issue and that I need to lose weight. It's so annoying to sit there and listen to everyone tell you the same thing over and over that you damn well already know. While you can introduce them to the subject and explain why, but stop egging it on.
Its our business if typically in the UK, where obese people claim it as a disability and claim benefits. For taxpayers funding these lifestyle choices, yes, it is our business, and its something that needs addressing.
What! A disability , that healthy people who actually take care of their self have to foot the bill , that's fucked up .. but hey there the ones who have no selfcontrol can't run,or really enjoy life and sit the fuck around typically have shorter life spans eventually the problem will correct itself.
It's not direct people get unemployment benefits to sustain their lifestyle however a lot of people don't use that money wisely ie getting alcohol instead of bread
And both groups have a negative impact on society. Yes, there are legitimate medical reasons someone could be obese but if you are actively hurting yourself and you know better, why should tax payers foot your health care bill? If you smoke, you should give up your OHIP
