I just ask their name because I have a terrible memory and I cannot see it as insulting.

Almost every time I admit to being horrible at remembering names, the person I'm talking to will admit the same thing.

Unless they have a really unusual first name, one where you probably wouldn't meet anyone with that first name before...

You could also ask them how their name was spelled or ask them to make a contact for you on your phone.
