As great as every actor has been to play the joker, Heath was the only one to characterise and humanise him, a man with serious mental issues but still a man. Everyone else's portrayal was very much just a crazy nut bag, humanity lost. That being said, I don't know if I and many other people would acknowledge that if Ledger was still alive.
The other part is that if you watch Ledger's performance, he never looks a person in the eye when he kills them. Nearly all other villains do, and they make it personal. But he just does it without casting a glance. And that sort of thing is terrifying for us, since even the remotest possibility of sympathy is lost in the action.
Actually there is a good theory about that by the film theorist in youtube... he says who the three jokers are (gang leader, actual clown and psycho) and says which one was portrayed in each movie, you should watch it i found it very interesting
