It's a control panel for his suit, controls things like oxygen, breathing apparatus, fluid intake apparatus and a mechanism to deal with bodily functions. It also has attachments to help him move about but the suit is very heavy and old tech so where his movement is limited he uses the Force to help him move about easier.

Also electro-magnets in his boots. I'd love to know what would have been different if Palpatine hadn't skimped out on Vader'a gear.
