Lets see, once that I know of in classic doctor who, once as eccleston, once as tennant, then there was the time in utah when he was killed, and all the places in america he went to during the impossible astronaut arc. Then he also was in modern day central park manhattan as well as old timey manhattan, during the angels take manhattan. Those are all the times I can think of, off the top of my head.
