I have seen some older homes with that in the States- but because we are evil little yanks and think of thinks to do to torture our neighbors like above less and less people have a direct opening into their home like that lol

On of the most visible issues that happened in the past few years when it came to issues of gender identity (at least the one I remember) is when Facebook was petitioned over and over again to offer more options than that of male/female, dual binary. That there were many people who existed outside of it (trans*, genderqueer, intersex, etc.) And many who existed without it at all. They expanded to 51 different options to their gender menu- it doesn't cover it all, but it's a great place to start:
