Yes, there are exceptions. But your comment truly adds to my point: the guy had to do exceptionally more than his main function of taking orders and bringing them to the table in order to get those tips. That may have been the etiology for the phrase, but meanings do tend to evolve. That being said, I think a portion of the taking religions out of schools and whatnot is an attempt to be more fair to people who are not Christian. As our country has evolved and become more diverse, it is only reasonable for this to evolve, as well. As such, not all of the removal of religion from schools and government is a result of "separation of church and state", but an attempt at more equality.
