Well, it's more like, it's possible anywhere, though the value of $10 changes depending on what part of the world you are living. Every state has it's own sort of macro-economy, which can cause the prices of things to vary per state, along with things like housing, and minumum wage. So, say in Kentucky, something could cost $10, but in Nevada, it may cost $15. What you can pay $10 for in Australia, may not be the same exact items and prices in many parts of America.

Me as a medic: I will try to heal anyone within my LIMITED RANGE. If I cannot get a fighting class to help escort me through the danger, and you don't come to me, then I will not heal you. If you run away as Im trying to heal, while switching between multiple people who all need healing, then I will assume you don't need heals that badly. If you start to complain how I am a terrible medic, just because I won't be pocketed by you, then I won't go out of my way to heal you. Yes, it's my job to heal, but it's everyone else's job to protect me.
