That's insulting in a way, where someone would pay $25k to get your face fixed, but at the same time extremely kind and would probably have saved the waiter a lot of money and happiness

He could probably see that the waiter was obviously uncomfortable with it. Im sure when he explained it he put it on the tabt.

Yeah I don't see it as insulting either. Like if he gave money and left a note to get a nose job or a face lift would be very different.
