Pinocchio's nose wouldn't grow, because he would say the truth then. But if the nose doen't grow, he's lying. So the only option is that his head is going to explode.
There's a difference between lying and being wrong. Lying requires knowing the truth. So does his nose grow whenever he says something incorrect or only when he does so on purpose? In the second case, no paradox, he's not lying he's just making a prediction that turns out to be false. However, if the nurse grows whenever he says something that is factually incorrect, then he'd possesses absolute knowledge of the entire universe, except for a few useless Gödel sentences like this one.
