We filled ours out in the hospital both times. But maybe that's just how that hospital does it?

We didn't have names right away both times so they just put "baby girl [our last name]" on all their records and when we picked out the name, they updated it all and we filled out the form. In Oregon, you full out a form and send it to the state and then request a copy of the birth certificate.

The whole birth process is pretty exhausting so she could be out of it anyway.
