The nurses do it to give doctor an idea of what's going on, but the doctor doesn't want to go on the exact words of the nurse because it's secondhand information, and still wants to hear it first hand.

Also, from my experiences, nurses write down what you say in order to keep record for the database. Where I live, anything the nurse puts into the computer goes to the database for the hospitals under that brand.

Yup i do this. We don't look at the chart. Nurse/ front office part is for records
