I've actually seen bikes with arm pedals. You are not missing out.

Some people in wheelchairs do have semi functional legs, and so they made some with pedals. Not only does it prevent atrophy, it's a huge morale boost!

Some people in wheel chairs CAN move their legs and even stand.

I had a friend with heart disease who could stand and walk for short periods but was otherwise stuck in the chair.
