I prefer the tried and true method of just talking really slowly and loudly. It always works when you have communication difficulties.

In my experience, gestures help a lot more.

I always find myself raising my voice when I have to deal with someone who doesn't speak English well, even if they're American and just have a stutter. Next time I'll try flailing my arms wildly as I shout obscenities and see how that goes.
