It's hard to keep track when you're cut editing so much

I don't know about editing so I'll take you at your word.
I guess it's a just a little thing often overlooked, but it's become automatic for me to count the shots. When they do it right I'm always little more impressed with the movie.
Well, you shoot a scene multiple times and you keep the shot that looks the best. Editor's have to go in and clear up audio and make sure it synched with the scene properly.
Directors might shoot a scene with a close up of a few characters and cut back and forth between perspectives OR they can do the same scene in a wide shot with less cuts.
With action scenes, it's very easy to confuse a take you did that day with one you did a few days ago and it's hard to keep perfect track of everything.
That's why movies have continuity errors all the time.
