Well tbf social norms weighed as heavily back then as they so today, especially in rich and powerful families. When you're supposed to marry your brother or cousin for the purity of blood to maintain status etc., you generally fukken so it, wether you like them or not. They didn't know it led to very messed up babies down the line, and everyone was groomed by this to the point of not putting it back in question. Now we consider this morally bad (hot take: I'm not sure why) so incest has become a very deliberate thing and at least people know they'll be judged.

For 4th cousins and beyond, the risks for birth defects dramatically decrease. So biologically speaking, it's alright to have kids with cousins as far away as that.
