Okay, I looked into it. The England holiday traffic in August 2023 that was disrupted was investigated and an enormous 23-points-for-improvement report was issued by the Civil Air Authority (CAA) in November 2024. It looks like the initial incident was the system getting "confused" by a international flight plan that used a completely normal location signal that the system interpreted as something else because aviation shorthand requires actually quite a bit of competence to get correct all the time. To be fair to the system: The worldwide aviation system was taken down accidentally pretty recently by a minor patch update with a bit of bad code.
