It's a reference number for a (very depressing) hentai called Emergence, where a girl gets bullied, addicted to drugs, raped, and many other terrible things. For some reason it has become a meme. (If you've ever seen the image where a girl is looking around her class and has a bunch of nice things written on her desk, I can tell you the original is not nearly as nice.)

I prefer the original picture. It says pain is relative, meaning that your concerns and hurts are just as valid as the next person's, no matter whose are "worse". Something a lot of people around me seem to not understand.

I recall seeing a response to this that stated it is indeed possible, but then everyone who used the water for bathing, plumbing, etc, would be committing sacrilege. So it is possible, it just shouldn't be done.
