It's not self entitlement you fool. It's assuming the kindness of a stranger based on their offer. The idea that a medical professional would offer relief in such a casual manner suggests that they were going to do so out of their own kindness. If they had not disguised their intention to charge the patient they would have simply said "We can provide you pain relief at a fee". That was not the phrasing they used. The entire transaction was predatory. The victim was not entitled, they were misguided. I hope your bitterness drowns with the rest of your unnecessariness.

Higher minimum wage laws need to be enforced so that people like your father should always be ensured that their hard work to put food on the table is fairly compensated. I'm glad your father was okay all things considered.

They have a monopoly on the market. Just like many other companies that participate in horrible labour practices, you can't go anywhere else because they are the gatekeepers to their industry. Also if you had to try and convince others to stop buying from Amazon you'd have to start a whole movement, which people have tried to do but as mentioned before where else do you go? Those movements always fail. The only option would be government enforcement of fairer labour practices and healthy competition legislation. This is a proven method of helping to morally "fix" crooked business' approach to labour. The capitalist ideal of "simply don't give them your money" is a frail attempt at stopping these practices and rarely works because someone always will give their money to them. I'm not calling for full government control of how a business runs, but the only way you ensure fair labour practices is if the government makes it illegal to run a business without adhering to them.
