I think the point is that she's eating and drinking healthily, so there's nothing wrong with her (despite the fact she out to be suffering from second-hand smoking).

Looks like the first one is smoking and his lungs are black as coal, the second one is drinking and her kidney is apparently dying, but don't know whats up with the last one.
