It sort pops out with out actually breaking the skin if you watch the episode "Partners in Crime" from Doctor Who you can see how it happens..

I always hated that episode. All those fat people, loosing a pound a day and loving it. And the little fat pieces becoming living babies to a race of beings that want babies. Then the Doctor comes along and ruins it and nearly kills everyone! Sometimes he needs to keep his nose to himself. I'd VOLENTEER to be an Adipose donator.

Here's the problem though, once you were taking the pills, EVERY type of tissue in your body could turn into adipose. What do you think those pendants were for? And I don't mean the one time Donna was jut absentmindedly playing with one.
