I feel 3 years old is a bit too young for a child to fully comprehend with what is going on. They are still practically a baby/toddler stage. A four or five year old sure but not 3.
My nephew is 5 and he hardly understands that I can see him hiding behind a sheer curtain during hide and seek.
Though he can beat portal 1 and 2 by himself and actually solved some puzzles some ways I wouldn't have ever thought of.
I have scars on my body from when I self-harmed, and I worked at a summer camp this summer. One of the girls I was working with one day saw, hugged me, and told me that I was very loved. Sometimes, kids are the best.
How adorable, a loving act from an innocent angle solved what grown ups spend years of study to get a uni degree to learn about it. Acts of love and care do more than a degree. (From personal experience )

It looks to me like he (correctly) judged that self-harm is bad and she should stop doing it.
