Let me start off by explaining 1 thing before I get to the scene in which "after all this time?" is said. The Patronus.
"This ancient and mysterious charm conjures a magical guardian, a projection of all your most positive feelings. A full, corporeal Patronus, is a guardian which generally takes the shape of the animal with whom they share the deepest affinity. "
It is also possible for your Patronus to change forms after a emotional upheaval.
In the scene we're talking about Snape is upset with Dumbledore for wanting to sacrifice Harry's life to defeat Voldemort. He (Snape) says that he's spent many years lying, spying and putting his life in jeopardy, and did it all to keep Lily's son safe.
Dumbledore is moved and asks if he's grown to care for Harry. To which Snape replies; " For him?!", and summons his patronus.

Snape's patronus has the shape of a doe. The same as Lily's. The implication here is that Snape's most positive feelings and that which he has the deepest affections for is Lily. The patronus in that sense is like a window into the soul. You cannot exaggerate it or falsify it. This patronus was the most honest and telling answer Snape could have given.
Dumbledore then asks Snape,
" After all this time?"
"Always," said Snape.
The biggest flaw i have noticed is why fred and george were never concerned about a "petter petigrew" in bed with their brother ron (seen on the marauders map) before harry discovered peter walking alone at night (who was of course, scabbers)
