Where as Harley is a Former Doctor?
(Yeah, I'm looking for clarification I have neither read hundreds of comics for Harley or seen the elaborate back story for Sparrow)
I don't think you can be a 'former' doctor, once you're qualified you're qualified. Retired doctors are still called Dr. The only exception is if she had her license revoked by the board, which is possible. Besides jack goes through periods where he has no ship and still calls himself captain
