Some people do have naturally thicker bones, like the giant teddy bear by the name of Andre the giant

I think when people say "big boned" they generally mean a wider/broader bone structure rather then thicker individual bones

Don't know, but the fat person's pic is a different layer, I think more to the back, as you can't see their spine in their neck. That also explains why their brains look different
