Doesn't have to be small, it could be well planned and laid out. Take the historical district in Savannah GA for example.

Small cities are likely to have poorer medical welfare systems and are likely to have lousy autopsy machines, poisoning with barium acetate will be much easier if the city doesn't have the autopsy machine to detect it
