Very interesting. May I ask the story behind the picture? Umbilical true knot is allready quite rare but in animals it's even rarer so it's a first to me
This lady had two fetuses in her womb. She had concieved through IVF and one of her babies was dead when she was 17 weeks pregnant so it was removed. This other baby got free space and rotated in a way that this knot was formed.
I looked it up: ““True knots” are knots that form in the baby's umbilical cord, occurring in roughly one out of every 100 pregnancies. They happen most often in identical twins sharing the same amniotic sac. They can also occur when babies flip and turn in the womb.“
I don't have any medical background, but I'd say pretty bad, as it seems to cut or at least slow down the blood supply.

This is pretty bad because as rightly said it compromises the blood supply plus the oxygen exchange which occurs through the umbilical vein which runs through the cord. The baby has to be delivered through Caesarean only and it is a high risk pregnancy. The baby can also die in her womb due to all these complications.
I'm googling but cant find the article that knows. They only talk about the first baby
