what irritate me more, was that it not only happened randomly, but the cord is winding the opposite way as well, and you couldn't wind it back. it just stayed that way.

It not random actually. Whenever we found one of our users with a phone cord that was all jacked up we would move their phone to the other side of the desk and it wouldn’t happen again. It’s caused by picking up the phone with one hand and then using the other to put it to your ear. When you pass it from one hand to the other you spin it without realizing it.
