Courage does not belong in that level. It is creepy but I was never afraid in fact I think it was the most hilarious show on Cartoon Network at the time. I think it's level 1.

Genuine question: I don't want to be that guy but what about his or his family's quality of life? It isn't easy taking care of or being in care of everyday.

The answer of how he lived long: medical advances providing people with the chance of extending their longevity. With all the love and patience of his family and the support of his friends gave him confidence to keep going. He had a purpose to live.
Now the dark side: if he didn't have any of that he wouldn't have lasted as long as he did.

My theory for that is Gizmo just wants to be loved. I'm assuming he spent all his life in a cage and always dreamed about being loved. Throughout the film he just wanted attention, I believe he was conscious and understood his nature but tried to control it. From where he is from I don't know, probably a failed experiment.
