I've looked up Sauron's physical form and there are most definitely allusions to it but I don't think there's any actual revealing as according to Tolkien. However Tolkien himself also alludes to but doesn't firmly confirm it.

The last alliance of Elves and Men? That was at the end of the Second Age. In the Third Age, the time of The Hobbits and LotR... some 3000 years later, the Elves are fleeing in droves.
They weren't fleeing, they had been slowly leaving Middle-Earth throughout those 3000 years as Men grew to become more and more dominant. There really weren't a lot of Elves left by the time of The War of the Ring.
