1st door is a formal thank you. The 2nd door is a friendly thanks because now we have a history together.

If you allow them to hold two consecutive doors for you (provided that you are not carrying things that occupy both hands), you are in the wrong.

It depends on whether the person let you through first or they entered and held the door behind them. I always try to reciprocate but it isn't always feasible.

I the UK we have "Thanks" for the 1st door, "Cheers" for the 2nd, "Ta" for the 3rd then extra awkwardness for any others.
