Just think about the fact that most American schools won't pay to have seat belts put in under the idea that having the seats close together will be enough

Depends on the district, more are putting in harnesses now, in my kid's district the buses have harnesses in any seat where elementary level kids sit which for most buses is in the front half of the bus and the kids are required to use them.

It's probably based on the (rather stupid) idea that older kids have more developed bones so they should need less defense

Probably just fell over. Something that heavy falling on its side would be a violent affair for the people inside
