Because the world is full of people who are dumb and are afraid of offending the fat person and think it’s okay to shame the skinny one. I don’t agree at all, if anorexia is in the DSM as a disorder because you put yourself on danger being so skinny then the opposite should also be true. The dsm should include the obese version on the opposite end of the anorexia spectrum.
anorexia isn't "oh this person is too skinny." its a mental illness to do with body image specifically the person thinking they need to get skinnier. Colloquial terminology of "reverse anorexia" is used for when someone is obsessed with their body image thinking they aren't muscled up enough not for someone that's fat.

The fat girl will be told that she is lazy and disgusting and she should lose weight....you know for health.
My point being, society tends to be kinder to non-fat people. There are a lot of people who take the whole "be healthy" thing as an excuse to be mean to someone who is probably already struggling to get through life.

You should just tell both of them for the sake of their health as both are mental and physical problems that need addressing by professionals. Both are as disgusting as each other IMO and neither should be societally accepted.
Because you are probably not their doctor, and how do you know they are not already aware of their weight/health issues? People who are struggling don't just instantly recover. Bodies take time to heal.
