"Afrixalus delicatus" - also known as Pickersgill's banana frog. I kid you not, they're called banana frogs, funniest thing I've ever seen. The leaf-folding part is pretty self-explanatory. Members of the "Afrixalus" genus (or leaf-folding frogs) lay their eggs on leaves which hang over water, often folding said leaves to better protect their clutch. (They're also quite talented at origami. But not at poker, as they tend to fold then leaf)
