Even if one partner has a vasectomy and the other has their tubes tied, you can still get pregnant. There is always a none zero chance.

Only if the ovaries are removed. If they are not, the egg can still get fertilized and create an ectopic pregnancy. It won't survive, but you can still get pregnant. Example provided - https://www.ncbi.nlm.nih.gov/pubmed/17267880
