Don't like this at all

My guess is that the wound was big enough to saturate the bandage and then blood seeped out through the "breathing" perforations. The person might also be on medication that helps thin out blood

You’re right @under_fire I’ve done this several times. Easiest way to do it is you put the fancy “breathing” band aid on too early and it just leaks or if you reopen a really bad wound
