Oh... Where do I start? Even when I reduce it to four letters, it's also in peril.
The list is endless, the amount of people who manage to butcher my name into all weird kinds of pronunciation and spelling have yet to reach saturation. They normally get about three of the seven letters right, but the order... Oh, the order...
,
Then they try to use my second name instead, which is a tad less disastrous, but still quite deserving of own mention.
,
For that reason, I cannot place the multiple disasters for fear of recognition.
My name's Claudia, I've been called Claudie, Julia and Maud. (if we don't count my mom who constantly switches around people's first names)
EDIT : the secretary of my department keeps calling me Charlotte now for some reason.
My first name is Eva (hence the username, it started as a parody of teenage girls in my country who thought it was cool to substitute v with w and k with q around the 2010s) which is usually fine but people get my last name wrong all the time.

Haha, I have a unique name even amongst East-Indian names. I live in Canada. If they get my name right within the first 2 times, they get a cookie.
