It's a sex joke. The pillow is to keep the bed frame from hitting the wall during vigorous activity.

A couple uole years ago, a girl I used to date called me to her house to have me help her with some computer stuff. She had installed the bottom left solution for her banging headboard since we had been together and I got all sad for a minute because 1) I hadn't thought of that and 2) she had clearly moved on
